New Plan Year Dates
Coverage runs July 1, 2026 – June 30, 2027. Open Enrollment is May 21 – June 2, 2026.
Three Medical Options
Choose from ELCOC Base PPO, Mid PPO, or High PPO — all with comprehensive in-network coverage.

Tips to Maximize
Make the most of your benefits this plan year.
Preventive care is covered 100% in-network on all medical plans.
Annual physicals, immunizations, and screenings cost $0 in-network.
Telemedicine PCP visits are 100% covered on every PPO plan.
Skip the waiting room — see an in-network provider via video or phone.
Dependents can stay on your medical plan until age 26.
Children are eligible regardless of student or employment status. In Florida, dependents may remain covered up to age 30 if they meet certain qualifying criteria.
Dental preventive is covered at 100% on both plans.
Two cleanings per year keep your wallet — and your smile — healthy.
$25,000 Basic Life & AD&D is provided at no cost.

As of July 1, 2026, Preferred Benefit Administrators is your medical claims administrator. Members continue to access providers through the PPO network.
Use the Provider Search page — it links directly to the medical, dental, and vision provider directories. Staying in-network keeps your costs lowest.
The Base PPO has the lowest premium and highest deductible, the Mid PPO is balanced, and the High PPO has the richest benefits and lowest out-of-pocket maximum.
Only with a qualifying life event (marriage, birth/adoption, divorce, loss of other coverage, etc.). You must report it within 30 days.
